If you make a mistake and want to clear the form press the
'Reset form' button.
Requests are usually played from 7pm nightly
or from 12-2pm on Thursday afternoon but please try and give
us at least 24 hours to process your request.
We cannot accept requests without you completing
all the boxes marked with an asterisk*.
Unless you state otherwise all details may be
broadcast not only within the hospitals but also on our DAB
Digital service throughout Tayside and North Fife. However
we are only allowed to broadcasts the first name of any patient
in hospital as part of the Trust's Privacy policy.

Ninewells Hospital (FM and bedside radios)
Roxburghe House (FM)
Royal Victoria Hospital (FM)
Our request programme is broadcast to the
patients on 87.7fm and Ninewells bedside
radios every evening from 7pm.
The request programme is also broadcast across
Tayside and North Fife on The Shared
Access Channel on DAB digital radio Sunday to Thursday.
This allows patients to submit requests for
their family or friends tuned in to The Shared Access Channel
on DAB Digital Radio.
Thanks to a donation on nine DAB digital
radios from shop@Panasonic Dundee patients in Liff and Strathmartine
hospitals can now also hear or programmes on The Shared Access
Channel.

every Thursday from 12-2pm.
Hear the Request Chart every Monday 12-2pm
on 87.7fm and DAB Digital Radio.
